To reserve your space, please complete the mandatory health screening form, then read and acknowledge that you agree and understand the liability waiver form before submitting it. If you would like a copy of the waiver for your records you can access that here.

This mandatory screening form has been created to provide us with initial information for potential participants who have shown interest in our meditations and to update the health/medication status of previous and current participants.  This information is completely confidential and is NEVER shared with anyone beyond the facilitators of our events.

We would never want to turn you away because of financial hurdles.  If you need a payment plan or need to explore another arrangement, please submit this form and then email info@followthesound.co with more details.

I understand that once I have registered for the event, my payment is non-refundable. If I need to cancel for any reason I will be given a credit to a future event, and I understand that I am responsible for my own schedule.
To maintain the safety of this work and in respect for all participants of the meditation, I agree to hold the details of this work confidential. I will not share with anyone the identity of those who are participating in the meditation. This includes maintaining confidentiality for all facilitators, organizers, helpers and healers who are also participating in the ceremony.
I understand that the facilitators reserve the right to deny my participation if they deem that it would be unsafe for me, for others, or for any other important reason that they deem appropriate. I understand that if this should happen before the start of the sound meditation we will refund the money completely.
I understand that the publication of texts, photo’s, sound recordings, videos are prohibited. No recordings or photographs can be made without permission of the organizers. No publications, interviews or details of any participant or locations may be given to any non-member, journalist or legal authority, without the permission of the organizers.
RELEASE OF LIABILITY READ CAREFULLY - THIS AFFECTS YOUR LEGAL RIGHTS In exchange for participation in the activity of SOUND CEREMONIES- EVENTS/WORKSHOPS & ANY RELATED SESSIONS/PROGRAMS and/or use of the property, facilities, and services of Anthony Pietromonaco and his Assistants, I agree for myself and (if applicable) for the members of my family, to the following: 1. AGREEMENT TO FOLLOW DIRECTIONS. I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Anthony Pietromonaco and/or his assistants, the employees, representatives, or agents of Anthony Pietromonaco. 2. ASSUMPTION OF THE RISKS AND RELEASE. I recognize that there are certain inherent risks associated with the SOUND CEREMONIES- EVENTS/WORKSHOPS & ANY RELATED SESSIONS/PROGRAMS and I assume full responsibility for personal injury to myself, my health, and (if applicable) my family members, and further release and discharge Anthony Pietromonaco for injury, loss or damage arising out of my participation, whether caused by the fault of myself, my family, Anthony Pietromonaco or other third parties in the experience. 3. PERSONALITY RESPONSIBILITY. I aim to accurately represent the information provided to me on or through this SOUND CEREMONIES- EVENTS/WORKSHOPS & ANY RELATED SESSIONS/PROGRAMS. I acknowledge that I am participating voluntarily in using or participating in the Training, Workshops, Events, and Programs, Products, and Services, and I alone am solely and personally responsible for my choices, actions, and results. I agree to use my due diligence and judgment before applying any recommendation or suggestion that I may receive on or through the event/workshop and any programs or sessions, or Services post the event/workshop. I acknowledge that I take full responsibility for my health, life and well-being, as well as the health, lives and well-being of my family and children (where applicable), for all decisions now and in the future. 4. INDEMNIFICATION. I agree to indemnify and defend Anthony Pietromonaco and any assistants against all claims, causes of action, damages, judgments, costs, or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family's use of or presence upon the facilities of Anthony Pietromonaco. 5. FEES. I agree to pay for all damages to the facilities of Anthony Pietromonaco caused by any negligent, reckless, or willful actions by me. 5. APPLICABLE LAW. Any legal or equitable claim that may arise from participation in the above shall be resolved under California law. 6. NO DURESS. I agree and acknowledge that I am under no pressure or duress to sign this Agreement and that I have been given a reasonable opportunity to review it before signing. I further agree and acknowledge that I am free to have my own legal counsel review this Agreement if I so desire. I further agree and acknowledge that Anthony Pietromonaco has offered to refund any fees I have paid to use its facilities if I choose not to sign this Agreement. 7. ARM'S LENGTH AGREEMENT. This Agreement and each of its terms are the product of an arm's length negotiation between the Parties. In the event, any ambiguity is found to exist in the interpretation of this Agreement, or any of its provisions, the Parties, and each of them, explicitly reject the application of any legal or equitable rule of interpretation that would lead to the construction of either "for" or "against" a particular party based upon their status as the drafter of a specific term, language, or provision giving rise to such ambiguity. Accordingly, the Parties specifically reject the application of Cal. Civ. Code §1654 to this Agreement, as well as any other statute or common law principles of similar effect. 8. ENFORCEABILITY. The invalidity or unenforceability of any provision of this Agreement, whether standing alone or as applied to a particular occurrence or circumstance, shall not affect the validity or enforceability of any other provision of this Agreement or of any further applications of such provision, as the case may be, and such invalid or unenforceable provision shall be deemed not to be a part of this Agreement.
